Meteor Forms℠ utilizes a straightforward three-step process that enables you to automate your paperwork.
Just send us your forms, we’ll build them; then you can embed your new Meteor Form℠.
1. Upload your paper forms
- Just upload your existing paper-based forms (if you want the PDF output feature)
- The blank forms you send us could be Word docs or PDFs
- The format should be 8.5" x 11" pages.

2. We will build it
- First, we will create the online form that you can use on your website
- Next, if you are using the PDF output feature, we will set that up
- Finally, we will configure the email notifications that are sent when a form is submitted
- If you need support, more design, or maintenance, we charge $85/hour (billable in 15-minute increments)
3. Embed your form
- Paste the embed code we give you on your website wherever you want the form to display

Many kinds. We accept various informational gathering forms, including surveys, quotes, contact information, and applications, among others. We cannot support illegal, adult, or gambling forms.
We currently do not accept online payment forms; however, this will be implemented in the future.

We would be more than happy to do the embed for you. We charge $85 per hour (billed in 15-minute increments) to handle support requests. This kind of request usually takes 15 minutes. We will need the password to access your website. Once we log in, we will attempt to embed the form for you. Most of the time, there is no problem; however, for more complex situations, we may need to spend more time on the additional work involved.
We provide you with a small code that the computer can understand.
You will copy the code and paste it into your website on the page or location where you would like the Meteor Form℠ to appear.
If you or your web person can’t do this, you can ask us to do it if you give us access.
Users can login, request support, download saved entries, and manage the account.
