Payment and Refund Policy

 

Services purchased on this website are subject to the following Payment and Refund Policies:

 

Payments

We accept all major US credit cards. All payments are subscription based. Payments are either monthly or yearly. The client prepays for each service period.

 

Cancellation

You may cancel at any time. Upon cancellation, your services are immediately terminated. We may terminate service for non-payment, system abuse, inappropriate usage and/or technical or billing problems.

 

Refunds

For cancellations due to system abuse or inappropriate usage:

No refund will be given.

 

For client cancellations on monthly subscriptions:

Cancellation during the first month of service; no refund will be given.

Cancellation during subsequent months; if cancellation occurs during the first 14 days in the monthly service period, half of the amount paid for the month will be refunded, otherwise no refund will be given.

 

For client cancellations on annual subscriptions:

Cancellation during the first month of service; no refund will be given.

Cancellation during subsequent months; the refund will be based upon the number of months remaining in your annual subscription. If cancellation occurs during the first 14 days in the yearly service period, 3/4 of the amount paid for the year will be refunded, if the cancellation occurs during the first half of the annual period, 1/2 of the amount you paid will be refunded, otherwise no refund will be given.

 

Timing and Publishing

After purchasing the service and uploading your blank forms, we may take as many as 5 business days to setup your form on our end. You will need to paste our embed code on your website to publish the form.

 

Shipping

This is an online service, no physical goods are shipped.
